What are the steps for MVA clients prior to care?

At Elevation Health, we aim to get you feeling, moving and performing optimally. This most certainly applies after a Motor Vehicle Accident (MVA). We are a multidisciplinary clinic dedicated to helping you get back to your optimal health.

If you live in Alberta, you are entitled to accident benefits coverage regardless of whether you were at fault or not. You do not need to see your medical doctor before starting treatment. You can get started with treatment right away by seeing any of our Physiotherapists. They are primary health care practitioners that can provide initial assessments and treatments.

At this time, we require payment at the time of service for all MVA-related appointments. Clients will then receive a receipt that can be submitted to their insurance provider or adjuster for reimbursement.

WHAT ARE THE STEPS OF MVAS?

Dealing with motor vehicle accidents is stressful and we want to help you. Below are the steps to make the process easier:

File Accident Report With Police

STEP 2

Contact your insurance company immediately

You must file both an accident claim AND injury claim. Then they will assign you to an adjuster.

  • You need to tell them you are filing BOTH claims because the adjuster dealing with the accident claim is different than the adjuster looking after your injury claim.

STEP 3

Book an Initial Physiotherapy appointment at our office

  • It is recommended that you book an initial appointment within 10 days of your accident to assess pre-approved treatment defined under "Diagnostic and Treatment Protocols".

STEP 4

Complete a Notice of Loss and Proof of Claim (AB-1 Form) your adjuster should send you this. Once completed, send back to your adjuster. This must be filled out prior to your first appointment otherwise you must pay upfront for your treatment otherwise you will be required to initially pay for your assessment versus the insurance company covering it.

Our practitioners will fill-out an AB-2 form and submit to your insurance company.

Provide the clinic with the following information as soon as it becomes available to you:

  • Name of the insurance company

  • Name of adjuster dealing with your injury claim and their contact information (i.e. phone, fax, email)

  • Claim number

  • The date of the accident
